Every year the Diabetes Coalition of Massachusetts recognizes
outstanding programs that contribute to the field of diabetes
prevention, education and management throughout the Commonwealth.

Eligibility:
DCOM's Programs of Excellence was created in 2006.  Each year the criteria for nomination can   
change slightly.
